"As a practicing dentist for over 20 years, few things have reignited my passion for patient care like Tosha and her course. Her program has truly elevated the standard of care in our office. What Tosha teaches should be recognized as the true standard of care in dentistry."
Tosha Kozloski, RDH
Healthier patients. Higher production. Same team, same hours.
The complete system for turning phase contrast microscopy into diagnosed disease, accepted treatment, and a hygiene department that runs like true healthcare.
- 6-month guided implementation
- Live coaching, not one-time training
- 20 CE credits + certification
- A complete clinical system, not a device
No pitch. No pressure. A real conversation about what is possible for your practice.
Four things that don't change, no matter the practice.
Visibility over assumption
The microscope shows what clinical intuition can't. Patients see it, they don't have to be convinced of it.
Protocols over motivation
Excitement fades after a training day. Systems are what keep a team diagnosing and communicating consistently.
Practitioner dignity
Your hygienists are healthcare providers, not tooth polishers. Every script and protocol treats them that way.
Whole-body honesty
The mouth is not separate from the rest of the body. We don't soften that to make it easier to hear.
I was the doctor who thought, there's no way I want to do that, too much work. I'd tried salivary diagnostics and it created this lag and confusion that frustrated my team and lost patients. The microscope changed everything. It takes just a few minutes, the whole team is invigorated, and we're stepping into our clinical authority in real time, showing patients something they had no idea about. I'm using antibiotics less. And the greatest takeaway was still the verbiage. Tosha is so good at giving you the exact words. It's her superpower.
Bleeding gums aren't a hygiene failure. They're an infection nobody's naming.
Mouths look "pretty clean." Bleeding gets wiped away before the doctor's exam. Nothing sounds urgent. "I just want a regular cleaning" ends the conversation, while real periodontal disease goes undiagnosed and untreated. Tosha calls this the watch-it loop, and it's the default pattern in most hygiene departments, not because your team lacks skill, but because nobody ever gave them a system to break it.
The microscope ends the loop. In about 30 seconds a hygienist takes a slide. In under 2 minutes, the patient is watching their own infection move on a screen. Seeing it removes the need to convince them. But a microscope alone doesn't change a practice. Teams buy one, get a short training, and it collects dust in the corner. What actually works is the full system built around it.
Two ways to run a hygiene department. Only one of them changes lives.
The Watch-It Loop
The TOSH Method
Six months. Three phases. One system that sticks.
Say It
The patient conversation scripts, the case acceptance system, and the documentation toolkit. Your team learns exactly what to say, in plain language, so patients say yes.
See It
Phase contrast microscope mastery and the perio diagnosis workflow. Your team learns the technique and gets a clinical decision tree that removes the guesswork on when and what to diagnose.
Systemize It
The production tracker, front desk alignment, and the patient demand creation campaign. This is where the system becomes permanent instead of a memory from a training day.
For the doctor. For the hygienist. Both matter.
For the Dentist
- ✓Higher perio diagnostic rate across the practice
- ✓Stronger SRP and gingivitis case acceptance
- ✓Predictable, growing hygiene production
- ✓Real doctor-hygiene handoff, not a guessing game
- ✓More periodontal treatment means fewer hygiene exams daily
For the Hygienist
- ✓Confidence diagnosing instead of just cleaning
- ✓Scripts that actually get a "yes," word for word
- ✓Recognition as the healthcare provider you are
- ✓A system that makes your day easier, not fuller
- ✓Patients who trust and thank you
The hygiene microscope came at the perfect time in my career. After years of treating patients through traditional methods, the bleeding gums never stopped. What a change for patients to see in real time the bugs causing their infection. This is a game changer for everyone's immune system and full-body wellness.
Everything your team needs to run this, nothing you have to figure out alone.

I was the resistant hygienist. Then one day changed everything.
Back in 2008, I started using salivary diagnostics, and it changed the way I practiced. I was getting good results, and I was busy. Really busy.
But I had this group of patients I never knew what to do with. They came in every six months with great home care, but they bled during their cleaning. They didn't have a dirty mouth. No heavy debris, no calculus. So I didn't fully understand why they were bleeding, and honestly, I wasn't sure scaling and root planing was the answer for them either.
So when the microscope showed up in 2010, I didn't want to touch it. I didn't think I needed one more thing on my plate. I thought I was doing just fine without it, and the other two hygienists in our practice were just as busy as I was, so they weren't using it either. It never even occurred to me that this was the thing that could finally show me what was going on with those patients I couldn't figure out.
Then one morning huddle, our doctor made us an offer: take a slide on every patient before lunch, I'll help if you need me, and if you still don't find it useful, we'll go back to how we were. We agreed, honestly expecting to prove him wrong.
By lunch, the three of us had collected $10,000 in perio treatment. Not because we pushed anybody. Because patients saw their own infection on the screen, leaned in, and wanted the care. It used to take me ten minutes to talk a patient into a lab test and periodontal therapy. With the microscope, it took less than two, because I wasn't talking them into anything. They could see it for themselves. And remember, this was 2010.
That day changed everything for me. But here's the thing: one good day isn't a system. I spent the next decade figuring out what actually makes it repeatable, the scripts, the protocols, the coding, the coaching that makes it stick past the first month, and I've refined it with practices across the United States, the UK, and Australia.
That's what the TOSH Method is. Not another training day. It's the complete system I wish someone had handed me years earlier.

Tosha Kozloski, RDH
A practicing dental hygienist since 2005, Tosha started using salivary diagnostics in 2008, years ahead of the field. In her best year in the operatory, she produced $440,000 working just 16 days a month. By 2010 she was co-teaching seminars on the systems and protocols she used chairside. In 2018 she moved into full practice management, learning front office systems inside and out, but her heart was in the mouth-body connection, so she came back to hygiene and founded TOSH, Teaching Oral Systemic Health, in 2020. She has personally trained hundreds of dentists and hygienists across the US, UK, and Australia. Everything Tosh Care delivers runs through her.
I had no idea I would go from cynical, burned out and exhausted by this profession, trying to find anything else I might be good at, to wanting to follow Tosha on this path.
Built for practices ready to change how hygiene works. Not for everyone.
This is for you if
- You're willing to build a schedule around treating patients, not just seeing them.
- Your hygiene department is underproducing despite a good team
- Your hygienists want to be seen as healthcare providers, not cleaners
- You already own a microscope that's collecting dust, or you're ready to purchase one and transform the way your patients see their health.
This isn't for you if
- You want a quick production hack with no implementation
- Your team isn't willing to change scripts or scheduling
- You're a high-volume PPO or DSO mill not ready to adjust appointment structure
- You expect overnight results with no coaching follow-through
If your team completes the training, attends the coaching calls, and implements the core TOSH workflows across the six months, and you still don't feel confident using the microscope as part of your periodontal diagnosis and patient communication system, I'll continue supporting your team until the system is fully installed.
Thank you for your role in my journey. Your encouragement, knowledge, wisdom, and kindness will forever be appreciated. Thank you for treating me as more than a client, I truly don't think I'd have advanced this far without you.
Tosha works with a maximum of eight active practices at a time. When those seats are full, enrollment closes until one opens up.

Answered honestly, before you book.
Is phase contrast microscopy evidence-based, or is this snake oil?
The microscope is used as a motivational and educational tool, and as an adjunct to your clinical diagnosis, not a replacement for it. It shows patients what's happening in real time, which is what moves them to say yes to treatment you've already clinically identified.
Will insurance pay for this?
The periodontal protocol packages include insurance coding built in for gingivitis, early/moderate perio, peri-mucositis, and peri-implantitis, calibrated to reduce denials and downgrading on procedures like SRP and gingivitis treatment.
I don't have time in my schedule for this.
Nobody does at first, and that's exactly the problem. Most practices I start with have their days booked prophy after prophy, with no room to treat the perio they're about to start finding. So that's the very first thing we fix. Before anything else, we build treatment blocks into your schedule, usually two 90-minute blocks a day, so there's real room to treat instead of squeezing it in or watching it walk out the door.
The diagnosis itself is fast, about thirty seconds for a slide and two minutes to show the patient. And after coaching dozens and dozens of teams, I know how to build those blocks in so your day runs smoother, not crazier.
My dentist won't fund this.
If you're a hygienist reading this, I see you, and I hear this a lot. You know your patients are bleeding. You want to actually treat them. And you're not the one who signs off on the investment.
Here's what I'd say. Most doctors aren't saying no to better care. They're saying no to what feels like a cost without a return. That's why the whole system is built to show the ROI up front: the program fee is roughly one full-mouth SRP case a month, and the goal is two SRP cases a day per hygienist. When a doctor sees that math, the conversation usually changes.
So start with a conversation with your doctor. And when they're ready to look at this seriously, book a call with you and your doctor together.
I've done CE before and nothing stuck.
That's the difference between a training day and this program. The TOSH Method includes six months of live coaching where your team is accountable to Tosha and to you, specifically because one-time CE doesn't change behavior on its own.
Does this come with CE credit or a certification?
Yes. Practices that complete the program receive 20 CE credits along with a phase contrast microscopy certification.
In their own words.
Practitioners and hygienists share their experience with the TOSH Method, straight from conversations on the Mouthy Matters podcast.
"I vetted many people to figure out how I was going to do this in my office. Then in walked Tosha, and we loved her. We have never looked back. Best thing I've ever done. Her program was a very systematic, down-to-earth, easy way of doing it. There was so much implementation that happened. From a practitioner's standpoint, she taught me what I wanted to know, and from a hygienist's standpoint, she really held their hands and helped them. Tosha also did a really good job helping us navigate the process with verbiage. We fine-tuned our processes so much that when a new hygienist joined us, it was plug and play. Implementing a microscope is by far one of the best things I've done in my 22-year career."
"I would recommend Tosha's program a hundred percent. It definitely changed everything from my viewpoints on hygiene. Most of my patients have come back and completely changed their oral health status because they're doing the things at home. I've had some patients who have always been bleeders who are no longer bleeding, and I thoroughly enjoy my job more. Thank you for all the information you've provided and for always answering my text messages throughout the day when I have questions."
"Tosha's program has had such a huge impact. We collaborate as a team now on an entirely different level. It's been so rewarding to see the healing and the changing, and the patients are now excited. It's also attracted a very different level of patient to my practice. I'm an entirely different clinician. I'm totally reinvigorated and excited, and it leads to an entirely different conversation during the appointment. Now I get to do dentistry in really healthy tissue, and it makes a huge difference restoratively. It's totally changed how I practice. I would certainly encourage other providers to consider adding this into their office."
"Tosha, I'm really glad you were our first coach as we were trying to implement and understand microscopy and salivary diagnostics. You really trained us well to be able to help cases like this. If we have a coach like you, you tell us, 'Focus here first.' There's just so much to incorporate, and you're not going to know it all, and it's okay. Looking at things visually is really powerful. It does make practicing very enjoyable. There's just so much that we can do, even as a GP, because people see us more than their MD. Thank you for what you do, for getting more of us to be able to do this."
"Tosha, working with you was a lot of fun. Whenever you started explaining everything, all of it made a lot more sense than it did before. The patient knows why they're getting treatment because they saw it, so there's trust and rapport between us. I feel more competent, more skilled, and confident. It's really changed my whole perspective as a practitioner, and I know that my abilities have just skyrocketed. If you're considering Tosha's program, just do it. It's intimidating, and you're going to have a million questions, but Tosha has an answer for all of those questions. When you start to see those follow-ups, that is when you know this was the right move."
"Tosha's Microscope Guided Hygiene program was pretty easy and straightforward, and we had fun doing it. It empowers not just myself and the patient, but the hygienist and the assistants. It was a really nice opportunity to learn and team build at the same time, throw around ideas for implementation, and help our team members feel heard and like they have ownership in it. The homework was not overwhelming, not at all. I don't think we've had a single patient say no to treatment since we started showing them the visual. It empowers patients to be in charge of their own health. There are few things I've done better in my career than taking that time. Our team now shares the same vision, which is passed down to the patients. They trust you, they believe in it, and they don't think you're just trying to sell them something."
Real conversations from the Mouthy Matters podcast.
Three practitioners who went through the TOSH Method, in their own voices.
Episode 19: Microscope Guided Periodontal Treatment, with Dr. Caroline Labritz
If you've been running saliva testing and still feel like you're guessing whether a patient is actually getting better, this episode is for you.
Tosha sits down with Dr. Caroline Labritz, a biological dentist in Morgantown, West Virginia, who has spent 22 years building a root cause approach to her practice. Before she found Tosha's program, she vetted multiple training options. What made her choose this one, and why does she say she'll never practice without a microscope again? That's exactly where this conversation starts.
Episode 18: From Lecturing to Partnering with Patients: A Dentist's Microscope Story
She used to show patients a perio chart and watch nothing change.
That is where this conversation starts. Dr. Laura Mach, a biological dentist in Marion, Iowa, spent years explaining bleeding gums to patients who nodded, said okay, and walked out having changed nothing. She could talk until she was blue in the face. It just did not compute.
Episode 17: How The TOSH Method Creates Dental Microscopy Results, with Dr. Vane
Dr. Nicole Vane spent years avoiding the microscope. Every mentor she trusted had one in their practice, and every time, she found a reason to skip it. Too much setup. Too much learning curve. The ozone would take care of it anyway, right?
Then she went through The TOSH Method, and it changed everything, for her, for her team, and for the way her patients understand what is actually happening in their mouths.
